One of the greatest gifts our loved ones can give to us is to carry out our medical wishes if we are unable to communicate them ourselves. Our time to provide clear guidance to them is now. Contemplating incapacity and death is difficult. Yet, it helps puts us in touch with our most authentic selves. It encourages us to be clear about our priorities. This clarity is critical to creating an effective Advance Health Care Directive (AHCD), by which we receive the treatment we have requested. In this webinar, estate planning attorney John O’Grady and hospice physician Dawn Gross will discuss the practical application of AHCDs, and how they are used (or not used) in various health care settings. We’ll review the essential elements of a solid AHCD; exploring how, when, and where to start the conversation; creating a practice of review to keep AHCDs current; and steps loved ones may take in the very likely event that an AHCD is not available at the time of a health care emergency.

Dawn Gross
Dawn Gross, MD, Ph.D., Co-Founder and CEO of Dyalogues, Inc, is an end-of-life care thought leader. A hospice and palliative medicine physician, her passion is to discover what matters most to her patients in this very moment. She completed her undergraduate education at USC; medical, graduate, and internal medicine training at Tufts; fellowship in hematology and bone marrow transplant at Stanford; and post-doctoral training at UCSF.
Dr. Gross transitioned to hospice and palliative medicine after her father died in 2006 and has become an internationally known speaker, educator, and consultant. She has worked with the UCSF Palliative Care team since 2010, and she is the medical director of ANX Hospice.
She is the creator and host of Dying to Talk, the first-of-its-kind, live call-in radio program. Her writing has appeared in The New York Times, JAMA, and Science and Annals of Internal Medicine, and her first book of non-fiction is forthcoming. Gratefully married for over a quarter of a century, she’s hoping that her three children eventually award her with a Master’s degree in parenting.
